Memory Care (Alzheimer’s & Dementia)

For families caring for loved ones with dementia or Alzheimer’s at home, it can be overwhelming at times due to the challenges of daily routines. Symptoms of the disease may be further complicated due to injury or illness. We help each person maximize his or her quality of life. Our goal is to provide a nurturing and therapeutic environment where your loved one can receive the clinical care and rehabilitation they need in a secure setting.

Memory Care Services Include:

  • Trained caregivers to care for patients with dementia, including those who have had an injury or undergone survey
  • Specialized Rehabilitation to include Physical, Occupational and Speech Therapies
  • Multi-sensory activities for each patient
  • Group activities and one on one activities
  • Mobility exercises
  • Help with personal hygiene and grooming
  • Nutritional and eating assistance
  • Private and semi-private rooms
  • Respite care
Post-Acute (Transitional Care Unit)

Those who have been stabilized in an acute care hospital are excellent candidates for post-acute care where they receive the continued medical and therapeutic management they require to achieve maximum recovery. Many of our patients have improved quickly and are discharged home or go to a more independent setting.

Outpatient Rehab:

We want our patients to be able to continue their progress with our rehabilitation team once they return home. We offer outpatient rehabilitation services to allow our patients to receive rehabilitation and continue the training and regimen they have become accustomed to during their stay with us. The goal of this service is to continue recuperation for long term results.

Our outpatient rehabilitation services are also offered to those who need rehabilitation in the community and are not already a patient at a center.

Stroke Program

Our Stroke Recovery Program  is designed to improve the quality of life of patients through education and symptom management. Our goal is to help each patient attain positive outcomes through evidence-based care management. We apply a multi-disciplinary approach to help our patients relearn daily living activities. With a variety of treatments and equipment we strive to if possible, discharge patients to their homes or to a lower level of care.

We treat a variety of conditions and provide an array of services including, but not limited to:

  • Stroke
  • Hemorrhaging Stroke
  • Transient Ischemic Attachs (TIAs)

Our Stroke Rehabilitation Services Include:

  • Physical, Occupational and Speech Therapies to Improve Strength, Endurance, Mobility, Ambulation, and Activities of Daily Living
  • Electrical Stimulation Physical Therapy (E-Stim) to Improve Swallowing by Strengthening Throat Muscles and Other Treatments for Swallowing Disorders
  • Barium Swallow Studies and X-Rays as Needed
  • Advanced, On-Site Equipment – Continuous Passive Motion Machines, Treadmills and Stationary Bikes
  • Perceptual Evaluation & Training, Memory Improvement, and Problem Solving
  • Bowel and Bladder Therapy
  • Psychological Counseling to help Understand and Adjust to Physical and Emotional Changes
  • Healthy Lifestyle Programs – Nutrition Assessments, Smoking Cessation, Stress and Weight Reduction
  • Family Conferences, Involvement, and Counseling
  • Home Assessments – Recommend Adaptive Equipment and Placement of Ramps & Railings
  • Discharge Planning
Respite Care

Respite Care provides time off for family members caring for someone who is ill, injured or unable to care for themselves.
Your loved one will be taken care of while you take that necessary break for holidays, vacation, wedding, birthday or even if you just need some personal time.
We provide a wide range of skilled nursing and rehabilitation services. Our focus is on improving the quality of life for those we serve through our orthopedic, neurological, cardiac, pulmonary, wound care services as well as interventions for other medically complex diagnoses, admitting 24 hours a day, 7 days a week offering both Private and Semi Private rooms.

* Some payers sources may require a minimum length of stay.

Respiratory and Pulmonary Program

Our Respiratory and Pulmonary Program is designed to improve the quality of life of patients through education and symptom management. Our goals are to apply a multi-disciplinary approach to reduce symptoms, increase participation in physical and social activities and, if possible, discharge patients to their homes or a lower level of care.

We can help care for and manage pulmonary diseases such as:

Wound Care

At Orianna Health Systems, many of our centers specialize in a Wound Care program.
